Description

An initiative to improve access to treatment and support services for youth and young adults ages 16-25 with a serious emotional disturbance (SED) or a serious mental illness (SMI). These services try to: Foster a partnership of providers who can empower youth and young adults to achieve their goals; Improve life outcomes for youth and young adults by eliminating barrier to accessing and participating in services; Increase functioning and capacity to complete activities of daily living.

Applewood Centers

Provides behavioral healthcare, special education, and social services for children, youth, and families. Offers a multi-disciplinary staff, including licensed counselors, social workers, teachers, nurses, psychologists and psychiatrists. Services include outpatient counseling, psychiatry, residential services, adoption, foster care, in-home and school-based services, internship programs and research partnerships. Provides the site for a residential treatment program for youth with both substance abuse and mental health issues. Operates five sites in Cuyahoga and Lorain Counties.
