Description

Offers social services information and referral for linkage to appropriate hospital and community resources. Helps with discharge planning activities to assess, plan and coordinate post-hospital care arrangements. Offers brief counseling to deal with crisis, stress and concerns that may accompany an illness, changed level in daily functioning, or dying. Offers creative problem solving education, and advocacy services. Offers critical incident stress debriefing.
