Eligibility

- Serves women age 18 and over in Stark and surrounding counties who are recovering from substance use disorders. - Includes women who are pregnant. - Children from birth through age four (4) are permitted to stay with their mother, with staff approval.

- Photo ID. - Social Security card. - Proof of residency. - Proof of income. - Medical or Insurance card, if applicable.

Application process

Call the office to register and to schedule an assessment appointment at the CommQuest location that is most convenient. The appointment takes approximately two (2) hours.

Fees

- Accepts Medicare, Medicaid, private insurance and private pay accepted. Call for details. - Offers a sliding fee scale according to income.
