Phone to make an appointment, no walk ins accepted. Phone for transportation information for required qualifications, must call at least 2 days ahead of time to schedule.

Fees

Fixed fee; Medicare, Medicaid, and third party insurance accepted. Those without insurance will be screened for Medicaid eligibility or Healthcare Marketplace. If ineligible the patient may receive an income based discount toward charges depending on county residence.
