Eligibility

Open to Ohio residents who are eligible for or currently receiving Medicaid. Participants must also be age 65 or older or have a physical, intellectual, or developmental disability and meet a Nursing Facility or Developmental Disability Level of Care as determined during an enrollment assessment.

Application process

A Medicaid waiver may be requested by:

  • indicating the request on an application for Medicaid
  • submitting a form to the County Department of Job and Family Services (if already enrolled in Medicaid)
  • making a request verbally or in writing to Ohio Medicaid
  • making a request verbally or in writing to an Ohio Medicaid approved long-term services and supports agency

Description

Oversees multiple Medicaid waiver programs for residents of Ohio who need ongoing support due to their age, a medical condition, or a disability. These programs provide special coverage through Medicaid for services and supports to be provided in an individual's home or community rather than an institutional setting and include the Assisted Living Waiver, MyCare Ohio, Ohio Home Care, PASSPORT, Individual Options, Level One, Self-Empowered Life Funding (SELF), and OhioRISE.
