Plan members who reside in Ohio. One membership per household.

Required documents

Health insurance card credit card to receive ongoing service Internet service and device that can receive Internet.

Application process

Call to get a promo code to register for the program. Those who are already members of the streaming service may also receive the discount. Code will be emailed to eligible members. Go online to sign up for the service or to apply the promo code to member's account. The hold on these reverifications has ended and reverification reminder notices will be rolled out throughout the year. Enrollees should look out for reverification packets in the mail and report any address changes to their JFS office.
