Up to age 19 and actively participating in a high school education.

Required documents

Photo ID and Medicaid or other insurance card are required at the intake interview. If custody is other than a parent, documentation of status is required.

Application process

Phone M-F: 9:00am-5:00pm for information. Callers will have an initial discussion to review admission criteria and make an appointment for an intake assessment (approximately 2 hours). Generally, appointments scheduled and completed within 24-72 hours. The consumer can attend treatment groups 1-3 days after intake. Can also check website for information and to send an email. Can also walk in M-F: 9:00am-5:00pm to fill out admission form and make an appointment but appointment will not be available on the same day.
