Eligibility

- Serves Medina and Summit County residents age 18 and older. - Individuals who reside outside of Medina and Summit counties will receive appropriate emergency medical care and will be referred back to their home county of residence for ongoing substance use disorder treatment services.

Provides a 24-hour entry point to treatment for substance use disorders. Services include the following: - Physical evaluation and level of care determination. - Care coordination to create a short-term treatment plan. - Recovery coaching. - Initiation of Medically Assisted Treatment (if appropriate). - Connects patients to local substance use disorder treatment services for ongoing treatment.
