Eligibility

Adults 18 and older who are terminally ill and have a life expectancy of less than 6 months. Focuses on serving older adults. Outpatient hospice serves residents of Eastern Cuyahoga County, most of Geauga County, and parts of Lake and Summit counties.

Certification from physician that the person is terminally ill.
