Serves people age 65 and over, eligible people with disabilities and people of any age with permanent kidney failure.

An online application. Other documents will be requested as needed.

Hours

- Initial Enrollment Period: Three (3) months prior to the month of age 65 and three (3) months after the month of age 65. - Special Enrollment Period: Eight (8) months after the last month of current work under an employer group health plan (not Cobra). - Open Enrollment Period (every year): October 15th through December 7th.

Assists walk-ins on a first come, first served basis as time permits between scheduled appointments.
