Eligibility

Support group is for adults age 18 and older.

Required documents

Proof of Insurance if applicable. Proof of Income and Family Size if sliding scale is desired.

Hours

M-F: 7:00am-3:30pm, plus 2nd Th: 7:00pm-9:00pm September through May for support group (however no meeting is held in December). Hours for classes are flexible based on specific needs, and may include evening and weekend hours.

Call M-F: 7:00am-3:30pm to enroll.

Fees

Support groups are free. All other services have fixed fees, but sliding scale based on household income and family size is available. Services are covered by some insurance.

Description

Provides knowledge and skills to help clients manage their diabetes. Services include inpatient teaching, outpatient individual and group sessions, phone consultation, support groups, and classes on topics such as acute and chronic complications, insulin and oral hypoglycemia medicine, testing methods, diet, personal hygiene, exercise, and psychosocial adjustment.
