- For utility assistance: Must have received shut-off notice.
- For rent or mortgage assistance: Must have received past-due notice or notice of eviction.
Required documents
Will be requested as needed and include:
- Social Security card, birth certificate, or photo ID for each family member.
- Proof of total household income and expenses.
- For utility bill payment assistance: Must provide shut-off notice.
- For rental or mortgage assistance: Must provide eviction or past-due notice.
