Must apply for PRC first and provide denial or proof that applicant has already used PRC. Income must be at or below 200% of the Federal Poverty Guidelines.

* Completed and signed application listing all household members, social security numbers, and dates of birth
* PRC Denial/Proof of Use
* Vehicle Registration in applicant's name (or other household member of driving age)
