Must be terminally ill. There are no specific income restrictions. Eligibility is determined on a case-by-case basis.

Application process

Call M-F: 8:00am-4:00pm for information or to make an appointment. Clients can self-refer, but if recently hospitalized it is recommended that a referral come from the hospital social worker, who can more easily provide necessary documentation. There may be a waiting list for inpatient services (not for in-home care).

Fees

Accepts Medicare or private insurance, depending on circumstances. There are provisions for patients without financial resources or insurance - phone for full information.

Provides a 37-bed inpatient facility as well as in-home hospice services for the care of terminally ill patients of all faiths. Services include 24-hour skilled nursing care, medications, medical equipment, physician visits, meals, chaplaincy services, beauty shop services, and other amenities.
