Eligibility

Residents of Cuyahoga County age 55 or older. Must meet the physical requirements for intermediate level of nursing home care.

Required documents

Medicare, Medicaid, or private insurance card if applicable medical records.

Hours

Day Health Center is open M-F: 8:00am-5:00pm. Most clients who are transported arrive around 9:00am. Home care is provided to help them get ready to leave for the center.

Application process

Phone and ask for Intake Dept. to receive a phone screening or to request a packet of information and/or a tour. Those who appear to be eligible will meet intake specialist either on site or in home.
