Eligibility
Employees injured on the job, who are required to have a physical exam (such as a DOT exam, Bus Driver exam, Medical Statement exam or OSHA mandated exam), who need immunization for possible exposures at the workplace or who are required to have a drug or alcohol screen.
Required documents
Photo ID. Any forms required by the employer. If getting immunizations, a record of past immunizations. If an employee was injured at work and initially treated at an urgent care or emergency department, he or she would bring the medical documentation from the visit.
