Open to those who are frail in the City of Euclid. Applicant must NOT have an answering machine.

Hours

Calls are made 7 days a week, once a day to each client. Office hours for contact are M-F: 7:30am-3:30pm.

Application process

Call to request an application, or pick one up at senior center or hospital. Prevention Officer will then arrange to review the application with client.

Description

Provides telephone calls to check on the welfare of eligible individuals. If no answer, emergency contact will be called, and if necessary a car will be sent to the resident's address with possible entry to the home if injury is suspected. Prevention Officer also calls each senior periodically to visit, and encourages them to call her if they are afraid.
