Eligibility
Youth testing ages are 4-17; adult testing begins at age 18. No restrictions on residency.
Required documents
ID card, insurance card, social security number. If applying for sliding fee scale, proof of household income and proof of residency.
Hours
M-Th: 8:00am-7:00pm, F: 8:00am-5:00pm.
Application process
Call to schedule an assessment to determine the level of service required. If appropriate, will schedule appointment. May have to leave a message that will be returned.
Fees
Medicaid, Traditional Medicare, some Medicare Advantage Plans, and a sliding fee scale program.
