Serves Summit County residents determined to have an ICF-MR Level of Care, which requires significant limitations in at least two (2) of the following areas: - Self Care. - Learning. - Mobility. - Self direction. - Capacity for independent living. - Communication. - Economic Self Sufficiency.

Description

Offers an alternative for persons who would otherwise be in an Intermediate Care Facility for people with developmental disabilities. - Transitions - Individual Options Waiver (IO Waiver) allows people to live in the family home, an apartment, or another setting with up to three (3) other people with disabilities. - Provider services for eligible individuals living in the family home.
