Eligibility
Homeless singles or families who are currently homeless as evidenced by living in a hotel, a car, tent, or other places not meant for habitation, or a shelter.
Required documents
None needed at time of referral. ID, SS card homeless verification required at intake. the case manager can assist the family in obtaining homeless verification.
