Eligibility

Adults aged 60 and older who reside in Cuyahoga County or ages 18 and older with physical/mental health disabiliities as well as eligible residents of Lake, Lorain, Medina, and Geauga Counties.

Required documents

Documents are not required, but the following information is needed: Medicare ID number, Social Security Number, list of medications, pharmacy name and location, annual or monthly income, dollar value of assets (including stocks, bonds, cash value of life insurance policy, savings, etc.) and the same information for a spouse, if applicable. All information is held confidentially.

Description

Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part-D, SSI, SSD, food assistance through SNAP (food stamps), HEAP, Golden Buckeye/Ohio's Best Rx Card, Homestead Exemption, prescription assistance programs, housing and many others. After completing the survey, individuals will be given a list of services for which they may be eligible. Benefits counselors will follow-up with the client and provide applications if needed.
