Eligibility

Up to 185% of federal poverty level for income requirements, must reside in the county and be in the women, infants or children category as described earlier

Required documents

Proof of household income, identification document for each individual, address verification, Medicaid card if applicable, proof of pregnancy document and formula prescription for certain specialty formulas.
