Eligibility

Open to individuals age 18 and older who have mental health and/or substance use concerns and are experiencing homelessness in Hamilton or Clermont County.

Description

Offers the Projects for Assistance in Transition from Homelessness (PATH) program. Provides outreach workers who visit with individuals who are experiencing homelessness and have mental health and/or substance use concerns in their communities. Outreach workers may provide information about and connection to mental health and substance use services and other supports.
