Eligibility

Males and females eighteen years of age or older. No restrictions on residence but must get to site on a daily basis.

Required documents

Proof of county residency: accepts a recent piece of mail (utility bill) with current address (No PO Boxes, shelters, or agency addresses) current voter registration card automobile registration pay stub or proof of employment seeking in the county recent tax return. Ohio Medicaid card and proof of ability to pay.

M-F: 6:00am-1:30pm, Sa, holidays: 6:00am-10:00am. Closed Sundays

Application process

Phone for an appointment/screening M-F: 8:30am - 5:00pm. Prefers but does not require photo ID for service. Clients should consult with their primary care physician (if they have one) to see what their options are concerning substance abuse maintenance treatment such as this.

Fees

Fixed fee. Accepts Medicaid-eligible individuals and those who can self-pay.

Offers a medically-supervised, holistic methadone maintenance program, which includes individual counseling, education, case management, laboratory urinalysis, crisis intervention and assessments. Those who only want opiate detoxification services will be referred to a hospital.

Operates a methadone maintenance/opiod agonist medication outpatient drug treatment facility for opiate and other drugs of addiction. Treatment includes case management, crisis intervention, laboratory urinalysis, methadone administration, individual or group counseling, medical evaluations, and a program for pregnant or postpartum addicted women which offers case management services and links to community resources. Provides alcohol and drug assessments as well as referrals to other appropriate programs. The center also offers educational and prevention programs targeted against alcohol abuse/misuse, urban violence, HIV/AIDS, other sexually transmitted diseases and tuberculosis.
