Required documents

I.D. for each family member Insurance card (if applicable) parental consent for youth. Proof of income if requesting a reduced rate: accepts pay stubs, assistance benefit letter, etc..

Hours

Primary care: By appointment M-F: 8:00am-5:00pm (closed 12:00noon-1:00pm). Same day appointments are available. Emergency open 24 hours a day, 7 days a week.

Application process

Phone for an appointment M-F: 8:30am-5:00pm (closed 12:00noon-1:00pm). For transport to women's center services: Call to arrange transportation from home to hospital appointments M-F: 8:00am-4:00pm.

Fees

Accepts most insurance.Third party payment accepted, including Medicaid, and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Those living further than 150 miles from the main site may have other requirements.

Description

Offers primary care and emergency room services, including a variety of specialties. When patients need help with prescription costs, will assist with finding and completing paperwork for drug company patient assistance programs. Will not fill prescriptions.
