Anyone who believes themselves or their family may be eligible for health insurance through Medicaid.

Required documents

Social Security number if applicable, dates of birth for spouse and any children If employed and insured through employer, policy number for current healthcare plan. It is helpful (but not required) to have an email account with username and password set-up before meeting with an assistor.
