Eligibility

Medicaid recipients age Birth through age 21. Including Pregnant individuals.

Required documents

No documentation is necessary. The Health Check form doesn't impact Medicaid eligibility. If the client completes and returns to the agency, then we forward to the Medicaid Insurance Managed Care Plan the client has for follow up. If this form isn't completed, then the client will only miss out on being contacted and provided information on medical facilities that are in network, who can meet the client need.

Description

Provides comprehensive and preventative services for children and youth who are covered by Ohio Medicaid. Lucas County JFS serves the client by forwarding the Health Check form to the Medicaid Managed Care Plan. The 2-paged form is a questionnaire that is sent out to households that include a child between the ages of 0-21 years or a pregnant individual. The form lists comprehensive and preventative services for the youth and pregnant individuals who are covered by Ohio Medicaid. If in need of any of the comprehensive or preventative services, then the Managed Care Plan will be in contact with the Medicaid recipient to follow-up on service inquiries.
