Application process

Referral must be made by Cuyahoga County Department of Children and Family Services (DCFS) or the Ohio Department of Youth Services, Cleveland Regional Office. Phone to request referral packet M-F: 9:00am-5:00pm.

Description

Helps at-risk teens make the transition from institution or foster family to living independently. Provides participants semi-independent living apartments and supportive services such as case management, education, job readiness training, life skills training, and counseling. Teens spend six months to one year in a totally supervised cluster site, then move on to a more independent scatter site.
