Eligibility
Adults 21 yrs and older who mentally competent and not a chronic abuser of drugs or alcohol. Permits will NOT be issued to felons or those currently being prosecuted for any felony or a violent or drug-related misdemeanor.
Required documents
A completed application form, passport size photos, copy of training certificate, and State ID with photo or Ohio drivers license.

Information/applications: M-F: 8:00am-4:00pm. Applications accepted: Tu: 8:00am-4:00pm, Th: 3:00pm-8:00pm and Sa: 8:00am-3:00pm.

Fixed fee for application and criminal background check. Only cash, money orders, or certified checks accepted. No personal or business checks.
