Eligibility

Eighteen years of age or older with a serious and persistent diagnosis of schizophrenia. Family members are also served. Covers Cuyahoga, Geauga, Lake, Lorain, Medina, and Summit Counties. Accepts Medicaid-eligible pregnant women.

Required documents

Proof of insurance if applicable. Proof of income if no insurance. Medicaid consumers residing outside Cuyahoga County may have to show additional proof to obtain coverage.

Uninsured individuals may wait several months for service. Phone intake M-F: 9:00am-5:30pm. May have to leave a message that will be returned.

Fees

Accepts private payments, limited third party payment (Medicare and Medicaid). Accepts Medicaid-eligible pregnant women before they are receiving Medicaid. Will work with individual pregnant women who become clients to submit Medicaid applications.

Description

Provides a team approach to outpatient management of schizophrenia that involves extensive patient education on the condition, diagnosis, medication management, support, and group and individual counseling. Service includes dealing with substance abuse issues. Groups are occasionally offered for family members. Medication education, monitoring, counseling, and treatment available for those taking the medication Clozaril.
