Eligibility

Serves adults 18 or older.

Required documents

First time patients must register and bring: Proof of income (W2 form/tax return OR 2 current pay stubs) Health insurance card (if applicable) Social security card Driver's license or Photo I.D. and Proof of Dependents (tax document or Social Security Card for any children). Bring any necessary paperwork from employer for work physicals.

Hours

By appointment Monday and Thursday: 9:00am-8:00pm Tuesday, Wednesday and Friday: 9:00am-5:00pm.

Application process

Phone for an appointment. Urgent needs may be referred to other organizations if unable to schedule. There may be a long wait on the phone.

Fees

Third-party payment accepted, including Medicaid and Medicare. Financial Counselors assess payment based on income for those without insurance. Sliding fee scale rates are applied based on income, scales goes to zero.
