Serves those age 13 and up who may be chemically dependent.
Required documents
Client Birth Certificate, Social Security Card, Health Insurance Card, Medications List, Custody Documents, Collateral Contact Information (Legal, Medical)

Parent/Caretaker Driver's License/State ID.
Hours
M-F: 9:00am-5:00pm, evenings and weekends as arranged. Closed all major holidays.
