Eligibility
Serves Cuyahoga County residents with Developmental Disabilities (DD) who are at least age 18 and are no longer enrolled in an educational institution.
Required documents
None. Individuals must have service plans developed before participating. Service plans will be developed at intake appointment and participants will be advised as to what documents to bring with them.

M-F: 8:30am-2:00pm.

Fees
Fixed fee paid by the Cuyahoga County Board of Developmental Disabilities (CCBDD) or other funding source.
