Application process
Referral may come from an individual, family member, family physician, caregiver or employer. A prescription of medical necessity is required for insurance coverage and reimbursement. Schedule an appointment by calling Audiology Services at 419-756-1133.
Fees
Accepts private insurance, Medicaid, Medicare, self-pay, and sliding fee. Hearing aids and assistive listening devices are not part of adjusted fee. Pre-authorization with an insurance company can be done prior to scheduling an appointment.

Description
Comprehensive hearing evaluations for infants, children, and adults. Diagnosis of degree and type of hearing loss for people of all ages. Hearing aid services include selection, dispensing, fitting, and repair and cleaning of hearing aids/assistive hearing devices.
