Eligibility

Must be 175% below the Federal Poverty Guidelines Utilities must be in the name of an adult household member

Required documents

Proof of income for each household member. Copies of most recent utility bills List of all household members, including social security numbers and birth dates. Proof of US citizenship/legal residency for all household members (can be SS card, birth certificate, US Passport, naturalization papers, permanent VISA, or INS ID Card. Do not submit original documents in the drop box.
