Eligibility

Ages 16 and older with documented disabilities.

Required documents

Completed application. Must be accompanied by a prescription from health care provider. Prescription must contain the date, name of person with disability, health care provider signature, state for which it is issued, a disability placecard and the expected duration of the disabling condition. A statement on health care provider letterhead with required information is acceptable in lieu of a prescription.

Hours

Hours will vary based on location. Generally, Mon-Fri 8 am-5 pm. Some sites do have extended and Saturday hours. Please call site or visit BMV website for specific hours.
