Eligibility
Individuals who are severely hearing impaired or individuals who are considered legally blind and are physically and mentally capable of handling or caring for a dog. There is no residency requirement - service is provided to individuals throughout the United States.
Required documents
Application, physician's form, names and addresses of four references.
Hours
M-F: 8:00am-4:00pm - Depending upon situation, training time takes about 3-4 months.
