Serves Columbia Gas of Ohio customers of Carroll, Harrison, and Tuscarawas counties with household income at or below 150% of the Federal Poverty Level.

- Proof of income for all adults living in the home or a zero income form completed and notarized. - Copies of most recent gas and electric bills, showing the last 12 months of usage. - Copy of a recent property tax bill, mortgage statement, or mobile home deed, if applicant owns the home.
