Eligibility

Serves families with children age 18 and under residing in Ohio with income at or below 90% of the Federal Poverty Guideline Level. Must also meet the following basic requirements: - Be a United States citizen or meet Medicaid citizenship requirements. - Be an Ohio resident. - Have or get a Social Security number. - Meet certain financial requirements. Changes in household composition and unreported income could all result in changes to benefit amount. Recipients are responsible for reporting these or any other changes within 10 days of occurrence. Receiving more than a household is entitled to could result in overpayment penalties or charges of fraud.

Required documents

Must complete an application and provide the following: - Verification of all income for the last month. - Written proof of pregnancy from a licensed medical profession (if applicable). - Any medical bills for the past 90 days. - Proof of any other medical insurance. - Photo Identification (driver license, state ID, school ID). - Proof of citizenship (birth certificate, hospital record, US passport).
