Eligibility
Applicants must be legally blind, at least 16 years old, physically and mentally capable of receiving benefit from a pilot/guide dog, and have previously completed Orientation and Mobility training.

Completed application/registration form (Completed online application, including Medical and Eye examination forms filled out by the applicant's medical provider and video submission.)
