Application process

Call M-F: 8:30am-5:00pm to make appointment. For transportation services, must call at least 2 days ahead of time to schedule.

Fees

Accepts Medicare, select Medicare HMO's, Medicaid, Medicaid HMO's, and many commercial/private insurance plans. Payment plans are available as is a sliding scale based on household income and family size.

Provides comprehensive medical care, dementia evaluation, fall assessment and management, geriatric consultation, medication evaluation, incontinence evaluation and treatment, and social services. Staffed by geriatricians, a geriatric nurse practitioner, a geriatric social worker, and a nutritionist.
