Eligibility

Adults 18 and older who are diagnosed with a chronic/severe mental illness and currently in treatment. Do not have to be receiving the Community Psychiatric Treatment Support Program case management services.

Required documents

Health insurance card (if applicable) Proof of income (such as W-2, pay stub, etc.), photo I.D., social security card.

Phone for an intake appointment or information M-F: 8:30am-5:00pm. Must wear a mask and adhere to social distancing guidelines. Adhere to Covid precautions.

Also provides community outreach, presentations for community groups, Mental Health First Aid trainng for groups and organizations.
