Programs include but are not limited to: PASSPORT, Assisted living Medicaid waivers, long term care consultations, caregiver support services, regional long term care ombudsman, home repair, healthy lifestyles and wellness programs, aging and disability resource center, Medicare Part D enrollment assistance. Additional services may include: legal assistance, case management, transportation, personal care, housekeeping, adult day care, home delivered meals, emergency response systems, and other community resources.
