Avocates for residents of long term care facilities. Assists nursing home and group home residents with asserting their rights and with problems concerning Medicare, Medicaid, and social security. Handles complaints of long term care services provided in home and community settings. Promotes volunteer visitation and advocacy services.

Provides assistance to families of children with special health care needs by helping them locate a network of quality providers and financial assistance to pay for the services needed.

Provides comfort, support, and care for families dealing with the last phase of life. Care team gives guidance and assurance with patients and families. Physical, spiritual, and emotional support is provided in the patient's home or where the patient lives. Palliative care is also available for those dealing with chronic illnesses, even if they are seeking curative measures. Bereavement services, including individual counseling, support groups, and workshops, are provided for the families of hospice patients, and also to anyone in the community living with grief.

A free family practice clinic with services in Spanish. The clinic is staffed by medical students and physician volunteers. All services are free and include comprehensive family medicine care, diabetes management, women's gynecology, rotating specialty care, and on-site lab draws. We do not offer services in obstetrics or dental care at this time.

Helps assure that consumers of long-term care services make the best decisions possible based on the most complete information available. Provides consumers with nursing facility survey information from the Ohio Department of Health, as well as verified complaint activity by Ombudsman investigators. Lists of all nursing homes, assisted living, and group, family and foster homes in the Summit, Portage, Stark and Wayne counties are also available.

Links eligible adults in need of dental care with no and low-cost services offered by volunteer dentists. This program is also known as the Ohio Partnership To Improve Oral health through access to Needed Services.
