Health Check | Lucas County Job And Family Services

Provides comprehensive and preventative services for children and youth who are covered by Ohio Medicaid. Lucas County JFS serves the client by forwarding the Health Check form to the Medicaid Managed Care Plan. The 2-paged form is a questionnaire that is sent out to households that include a child between the ages of 0-21 years or a pregnant individual. The form lists comprehensive and preventative services for the youth and pregnant individuals who are covered by Ohio Medicaid. Community Health Clinics | Knox County Community Health Center

The Health Center is a patient-centered medical home offering preventive and primary care to patients of all ages. Medical services will include, but are not limited to: treatment of illness such as ear aches, colds, sore throats, sinus infections and management of chronic disease such as diabetes; physical exams for work, school and sports; well-child exams; immunizations for children and adults; and women’s health including pregnancy tests and Pap tests.
