Neighborhood Navigator | Summit County Public Health

Offers central intake for expectant women and families to provide resources and coordinate assistance by community health workers (CHW) in the client's preferred setting (home, agency, and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid. Completes a minimum of three (3) outreach attempts on each person screened to ensure all needs/opportunities have been met.

Help Me Grow Home Visitation Program | Catholic Charities Lorain County

Offers regular, planned home visits to pregnant women and families with information on resources and skills to help them raise children who are physically, socially, and emotionally healthy and ready to learn. Offers help from health, social services and child development professionals. Home visits may include: - Supporting preventive health and prenatal practices; - Assisting mothers on how to best breastfeed and care for their babies; - Promoting parents' use of positive parenting techniques; and - Working with parents to set goals for the future, continuing their education, and find employment and childcare solutions. Help Me Grow Home Visiting | Washington County Family And Children First Council

Help Me Grow Home Visiting is Ohio's program for expectant parents, newborns, infants and toddlers. The program provides health and developmental services so that children start school healthy and ready to learn. Help Me Grow works with parents to support young children's development. Home Visitors provide on-going home visits in the home; focus on improving prenatal health habits and infant care giving skills; provide information on child's health, growth, education and development through the Parents As Teachers program; provide screenings for health, hearing, vision and development; connect families with community resources and services. Affiliated with the evidence-based home visiting model Parents As Teachers. Services include - Developmental Screening - Information on parenting tips - Assistance in developing and reaching family goals - Home Visits to help each child stay "on the road" to healthy growth and development - Service Coordination - professional nurses, teachers, or other qualified staff who share their knowledge about available resources, assist families in connecting with needed services, coordinate services to help families meet their needs and goals.
