Home visits are available for tuberculosis and respiratory patients. For complete homebound patients, home visits are available including, flu and pneumonia vaccines, medication assistance, air conditioner and respiratory equipment (with a physician’s order). Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently)

Provides a variety of services that enable individuals to remain independent and cared for at home. Included are planning for families with a member who requires an increased/increasing level of care; recommendations and referrals to address problem areas; counseling to deal with life change, loss, anxiety and depression; physical, occupational, or speech therapy; skilled home nursing; personal emergency response systems that alert a remote agency when someone has fallen and requires assistance. Service includes caregiver training.

Provides many skilled nursing services to our clients, including wound care, infusion therapy, supportive services are provided by professional aides and companions who give comfort and support to in-home clients on a day-to-day basis. Our companions will assist with shopping, cooking, reading, errands, light housekeeping and will provide transportation to doctors' visits. Most importantly, our home care aides provide the needed companionship to clients and loved ones during physically and emotionally difficult times. Provides Medicare certified and private duty home health care services. Will work with the primary physcian for medication management, post operative care, wound care, diabetic teaching, COPD and catheter care, and other skilled needs. After skilled services have ended may assist with meal preparation, light house keeping, medication reminders, bathing, and providing companionship. Also provides Palliative care - a multidisciplinary approach to specialised medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms, pain, physical stress, and mental stress of a serious illness—whatever the diagnosis. The goal of such therapy is to improve quality of life for both the patient and the family.

Offers home health aide and home nursing services, including RN/LPN, case management, medication and disease process management, patient/caregiver education, IV therapy, wound management, ostomy care, home health aide services, physical therapy and speech therapy. Also offers personal care and light housekeeping.
