Program designed for adults with severe and persistent mental illness to learn basic life skills as well as further their education and employment goals.

Temporary living environment where treatment and support services can begin. Six beds; not a crisis unit; length of stay two weeks, up to one month. For persons with mental health problems who are homeless or are in danger of becoming homeless, or need to acquire skills and access to resources to achieve independent living.

As individuals approach their senior adult years, their Service and Support Administrators (SSAs) can assist them with resources and supports in such areas as: Home modifications that can allow an individual to remain more independent and safe in their home Benefits planning related to Medicaid, Medicare or SSI Retirement and financial planning resources Staying active in the community and engaged in meaningful activities Connecting with resources from the Area Agency on Aging, the Coshocton County Task Force for Older Adults or Coshocton County Senior Center Timely medical care, in-home health services, physical therapy etc. Helping individuals/team locate assisted living or nursing facilities for rehabilitation or long-term care as needed

Administers home and community-based supports for older adults and those with disabilities. Programs include but are not limited to: PASSPORT, Assisted living Medicaid waivers, long term care consultations, caregiver support services, regional long term care ombudsman, home repair, healthy lifestyles and wellness programs, aging and disability resource center, Medicare Part D enrollment assistance. Additional services may include: legal assistance, case management, transportation, personal care, housekeeping, adult day care, home delivered meals, emergency response systems, and other community resources.

A Service and Support Administrator supports individuals in determining and pursuing life goals by providing coordination across all life areas and across all systems. They assist individuals in obtaining these goals by: - Determining eligibility for services - Assessing individual needs and wants for services and supports - Developing “person centered” Service Plans - Exploring available funding resources - Assisting in the selection of qualified service providers - Serving as a resource Services and Supports is holistic in nature and addresses the individual rights and responsibilities as a citizen, participant, and contributing member of society.

Assists consumers in cases where an immediate change in residential setting is needed, where it is necessary to deal with law enforcement, or where there is alleged abuse or neglect of the consumer. Provides information and referral for all services. Monitors residential and day programs, does complaint resolution, and acts for advocacy and protection of client rights. This office also monitors Board compliance with legal regulations, oversees its accreditation, and evaluates its services in terms of goals and outcomes.
