Legal Assistance And Case Management - Domestic Violence Survivors | Blossom House Bureau Of Domestic Violence

Offers assistance in dealing with the justice system to individual victims of domestic violence, stalking or related crimes who are looking to file protective orders. Includes information on the legal process, accompaniment to court appointments, safety planning, and planning for next steps in the legal process. Advocates are not attorneys but have access to lists of attorneys who are knowledgeable about domestic violence. Also provides case management for survivors of domestic violence.

Case/Care Management For Homeless People | Welcome House

Offers a variety of supportive services to those experiencing housing instability, including assessments, case planning, financial education, budgeting assistance, housing counseling, and employment support. After participants obtain stable housing, they may receive in-home follow-up visits from a Service Coordinator for up to six months.

Community Psychiatric Support Team | Womensafe

Assists individuals in gaining access to a range of services available to them in the community, such as housing, income assistance, safety, physical and behavioral health needs, parenting education and vocational needs. Develops an Individual Service Plan (ISP) with the client and works with victims of domestic violence on issues of self-esteem, life-skills, and safety planning for themselves and their children. Also provides reestablishment services, such as relocation assistance and emergency rent/mortgage payments, security deposits for those with financial need. Often provides needed household items such as furniture, cleaning products, personal hygiene products, clothes, food, etc. Maintains follow-up contact with clients for a minimum of three months to two years.

Provides case management for individuals with compulsive hoarding issues for older adults and individuals with disabilities. The Bureau for Children with Medical Handicaps (BCMH) is a health care program for children ages birth-21 years. BCMH links families of children with special health care needs to a network of providers and helps families obtain payment for the services their children need. Children with special health care needs are defined as those who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services of a type or amount beyond that required by children generally.

Pace Care | Mcgregor Pace

Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Pathways Hub | Lorain County Community Action Agency

Offers empowering support to pregnant women throughout their pregnancy and up to their child’s first birthday. The Community Health Worker assists the pregnant woman in navigating the health care system and social services as well as educational support throughout each trimester of pregnancy. Services are individualized and adjusted for each woman's needs. Also provides diapers, wipes, training pull-ups and period products. Sizes vary and may not always be available. Assists in developing a collaborative plan of action for multi-need children and adolescents upon referral. Provide a standardized screening and assessment to determine appropriate levels of care. Case management may be provided to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs. Provides counseling to help youth achieve substance abuse and alcohol treatment objectives. Inall cases, the child's family will be included in the assessement, case management, and counseling process.
