Family Stability & Shared Youth Placements | Greene County Family & Children First

The Family Stability & Shared Youth program provides service coordination and support to Greene County families having children receiving services from multiple service systems. The program also assists the families and agencies in determining the need for placements, identifying options, coordinating funding, monitoring progress as well as discharged planning from residential treatment centers.

Juvenile Court Family Resource Center (FRC) | Summit County Juvenile Court

Develops partnerships with parents, guardians, and youth in order to strengthen the family unit by providing case management and supportive services. The goal is to help youth succeed in school and develop life skills with help from the family. The Family Resource Center also aids families that are dealing with issues such as domestic violence, truancy, teen pregnancy/parenting, job readiness, and youth who are demonstrating at-risk behaviors.

Provides a Resource and referral assistance and general case management program that assists families struggling with challenges and assistance identifying and working towards goals. Next Steps College Completion program pairs Success Navigators work with low-income students at Sinclair Community College to help them overcome non-academic barriers to completing their education.

Assists families that have multiple needs navigate services. Provides the venue to address family needs, eliminate duplication and build natural supports. Assists with connection to family supports such as respite, camps, social activities, coaching, parent education, advocacy and assistance for paying the cost of these services. Transition planning provided between home to residential placement and back to the home and community. This includes determining the goals of placement, reviewing progress made and supports needed to bring the youth home and assure a stable environment. Structure includes a Service Coordination Steering Committee that reviews cases, approves use of funding, and monitors outcomes.

Family Assistance Program (FAP) | Muskingum County Board Of Developmental Disabilities

The FAP program provides respite care; counseling, training and education; special diets; special equipment and home modifications. In addition, the new FAP program covers expenses for individuals on various waivers for any of the above services not included in the waiver. As with the old program, FAP is a payer of last resort where families are required to exhaust other possible funding sources before FAP will cover services. Available funding is based on a sliding fee scale and the family is required to furnish documentation of the families income of the last reported tax year.
