Family Stability & Shared Youth Placements | Greene County Family & Children First

The Family Stability & Shared Youth program provides service coordination and support to Greene County families having children receiving services from multiple service systems. The program also assists the families and agencies in determining the need for placements, identifying options, coordinating funding, monitoring progress as well as discharged planning from residential treatment centers.

Wraparound is a planning process which draws upon the strengths and resources of several to embrace individual needs embrace individual needs and surround families with the support to help maintain children and youth in their homes and communities. It is a free, and voluntary, service to any Preble County family that needs assistance navigating multiple agencies and resources to meet the needs of their child.

Wraparound And Service Coordination | Lorain County Children And Families Council

Offers Wrap Around services, an intensive, strength-based, individualized care planning and team management process that focuses on voice and choice of the family to meet their identified needs as agreed upon by the team. Each family is assigned a certified Wrap Around facilitator who will guide and support families through the process while facilitating the development of a team of professionals who will work along side of the parents and their natural supports within regular team meetings.  A written plan is developed and monitored monthly by the team to assess family progress against goals for improved family functioning.  In addition, all families are offered a family advocate who can offer additional support by representing the family voice when requested or when the family is facing various relationship challenges with system involvement.

Community Support Teaming | Licking County Children And Families First Council

Community Support teaming is available to all families with youth ages 0-21 who have multi-system needs, are at risk of out-of-home placement, or are returning from out-of-home placement. High Fidelity Wraparound Team Facilitators will assist the family in building a team, coordinate services with providers working with the family and provide linkage to community resources in an effort to ensure sufficient support is in place for the child and their family. Community Support Teaming is committed to strengthening families and improving the local social and human service delivery system by sharing resources and developing and coordinating services and resources for children, youth and their families.

Provides planning and coordination of services for families whose children face multiple challenges. Works to develop, implement and evaluate coordinated responses of care with local and government agencies to resolve child and family issues.

Family Conferencing | Licking County Court Of Common Pleas

Serves youth who are unruly and do not meet criteria to be involved on a Diversion level with the Court but they and their families are struggling with a multitude of challenges. Conferencing seeks to link natural supports and community services in an effort to divert the youth from official court involvement and to provide the family with services that will reduce the unruly behaviors of the youth. The focus is on promoting self-determination and skill building providing a forum for the family to resolve their own issues. Through family conferencing, a mediator and therapist work to facilitate a process that will pull the natural support system together to problem solve and develop a family plan that makes sense to the culture of the family. The family is provided support, encouragement and skill building and/or referral and linkage to other community supports.
