Offers homemaker service to clients ages 60 and over. Homemakers come to the home and perform light housekeeping, laundry, grocery shopping, necessary errands, etc.

In-home client assessments link individuals with appropriate long-term services; services are an alternative to nursing home placement which include but are not limited to adult day care, home-delivered meals, chore service, homemaker, minor home modifications, medical transportation, personal care, social work counseling, nutritional counseling, emergency response, independent living assistance, medical supplies and medical equipment.

Includes waiver-Medicaid, homemaking and aide care.

Provides aides to complete homemaker tasks such as light housekeeping, grocery shopping, laundry, seasonal deep cleaning and errands (prescription pick-up, etc.). While in the homes, staff assesses each situation to ensure safety and cleanliness. Each individual has a personalized plan of care to outline their needs, which is updated with the participant on a regular basis. Persons receiving the assistance are asked to provide supplies necessary to complete the tasks requested.

Homemaker Individuals assist with light housekeeping, meal preparation, grocery shopping and other errands. Companion Caregivers share time together, participate in activities, supervise and assist with meal preparation. Social Services Medical social workers assist with financial and social needs and help in connecting with community resources.

Includes services such as full house cleaning, scrubbing floors, bathroom and shower cleaning, dusting, vacuuming, laundry, grocery shopping, and cleaning cupboards. Will also pack, unpack, and organize home during a move. Will not clean outdoor windows requiring the use of a ladder or perform full carpet cleaning.

Provides in-home light housekeeping to assist with maintaining independence. Services provided by Rent-A-Daughter Senior Care Services. Billing handled by Community Partnership on Aging.

Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.)
